Position Summary
The Hospital Follow-up Coordinator is responsible for scheduling all appointments for patient hospital follow-up.
Job Description
-
Schedules hospital follow-up appointments according to the provider requests, addresses clinical questions in coordination with nursing staff and chemotherapy scheduler

Preserves up-to-date patient demographic information during scheduling by collecting any change in data as well as temporary address if needed

Verifies insurance coverage and communicates with Financial Counselor / Billing department to ensure proper billing information, referrals, and approvals are available for first patient consultation

Assists physician’s in preparing for a patient’s visit by obtaining and reviewing all patient records form hospital stay and relevant documents per hospital notes (i.e. test results, lab work, etc.), and import into the EMR (electronic medical record)

Communicates directly with patient to ensure attendance and minimize no show rate for hospital follow up appointments

Follow-up on patients who no show or cancel appointments repeatedly per practice policy with appropriate documentation to the referring physician office

Coordinates scheduling of patients who call into the facility or need to reschedule an appointment

Responsible for patient lost to follow-up calls in a timely manner

Responsible for assisting New Patient Coordinator and 208 scheduling line as needed in absence

Performs and complete other related job duties or project as assigned by management staff

Preserves quality services by following organizational standards

Maintains patient privacy by keeping all information found in patient files confidential; HIPAA compliant
